Primary care CKD review templates

What is this?

• A primary care template in SystmOne and EMIS that can be used before, during and after reviewing a patient with CKD.
• A multidisciplinary team of primary care leads in Renal, CVD and Diabetes co-designed the template with support from a consultant nephrologist and a clinical nurse specialist in CKD.
• The templates were created and approved centrally by NWL IT, following local SystmOne and EMIS board approval.

What it does

• The first page of the template contains coding and treatment guidelines to support clinicians to manage patients according to the latest guidelines.
• You can use the CKD template before scheduled reviews to prepare for the appointment, and during appointments to guide the discussion and make sure the patient is being offered optimal care.
• The second and third pages of the template can be populated with patient and clinician education links that are relevant to your local area.

How you use it

1. SystmOne and EMIS users can download the template to their computer and then import it to their own system to use for reviewing patients with CKD.
2. Use the CKD template codes to create your own template in your SystmOne or EMIS system.
3. Use this template as part of a CKD review or during a type 2 diabetes or hypertension review of a patient who also has CKD.
