Psychosocial and Rehabilitation
Purpose
Working with multi-professional colleagues from across London, we will identify a ‘portfolio of areas’ with which the multi-professional team can help, for example pain, fatigue, dietary management of symptoms and pharmacological management of symptoms. We will develop resources such as pathways of care and education, and work with colleagues in other priority areas to include and embed these in their workplans.

Our Workforce…
There is a wide variation in the provision of psychosocial support and rehabilitation for kidney patients in London. We have pockets of innovative practice and good care but for many Units services are stretched or lacking. There are unique workforce challenges associated with this relatively small workforce, which extend beyond recruitment and retention. Visibility and recognition of roles and scope of practice have been highlighted by the multiprofessionals. Despite the challenges we have an engaged and proactive multiprofessional workforce in London who are passionate about working together to improve psychosocial and rehabilitation for kidney patients in London.
Aims and objectives of the group:
Aim
All kidney patients in London who would benefit from accessing multi-professional team member assessment and intervention are signposted to the right professional(s) in a timely manner
Objectives
- Develop the LKN MPT (Multi-professional team) Forum to become a single point of access for expert opinion on how the MPT can support kidney patients and how that support can be embedded in pathways
- Develop MPT intervention pathways to support management of complications and common symptoms of kidney disease
- Each LKN priority work area has MPT interventions embedded within their priority objectives for 2025-26 and beyond
- Review the support available to help people live well with CKD, focusing on psychosocial support and physical function including rehab and prehab
