Renal 3P Transformation Event 21 July 2025
NHS England London invested £15m over 3 years to fund ICB-led improvements across the full kidney pathway from early detection though to supportive care. This event:
-
- Shared learning from all projects across all 5 London ICBs and the renal units, through focused presentations and poster sessions
- Provided opportunity to plan how to further develop the services, through themed workshops
- Encouraged development and building of relationships across teams, units, providers and ICBs
Themes from the day

- Emphasis on system working and neighbourhood-level care
- Blood pressure control is a clinical priority
- There is an increasing use of data (e.g. CVD Prevent) to drive improvement
- There is a need for practical tools to help primary care manage CKD optimally
- Patient activation and self-management featured prominently
- Projects addressed health literacy and communication barriers
Feedback was highly positive with attendees valuing the opportunity to share evidence of impact and to learn from others
