What is the purpose of this workstream?

The Young Adult Group works to address the unique needs and challenges faced by young adults with kidney disease. The team focus is to improve the transition of young adults with kidney disease from paediatric to adult care. It aims to streamline the transition while offering the best of care.  This includes addressing key areas of concern, such as medication management, psychosocial support, and training in providing developmentally appropriate care for all healthcare professionals. By providing a seamless and coordinated transition process, the YA team aims to help young adults with kidney disease achieve better health outcomes, improve their quality of life, and successfully transition into adult care.

‘Young adults with kidney disease face unique challenges when accessing healthcare, and while there are some examples of excellent practice, for too long this area has not been given the priority it deserves. We hope that our group’s work will help improve outcomes, and ensure that every young adult with kidney disease in London has access to best practice developmentally appropriate care.’

Aims and objectives of the group:

Aims

    • The London Kidney Network Young Adult team aim to improve the holistic care of Young Adults (16-25 year olds) with kidney disease across London. 

Objectives
 

    • Ensure that every young adult with kidney disease in London has access to a dedicated multidisciplinary Young Adult clinic.
    • Establish a system of routine data collection for transition care and patient experience.
    • Ensure all health care professionals who have contact with young adults are appropriately skilled to provide best-practice care.
    • Enable young adults with kidney disease to have psychological support as part of their kidney care.
    • Ensure that young people are put at the centre of their own care
    • Work towards reducing health inequalities among young adults with kidney disease in London
