SystmONE CKD Dashboard
What is this?
- Searches available in EMIS and SystmOne
- Criteria were agreed by a multidisciplinary team of primary care leads in CVD, renal and diabetes, with support from a consultant nephrologist and clinical nurse specialist in CKD
- Searches were approved and created centrally by North West London (NWL) IT after local SystmONE and EMIS board approval
What it does
Enables primary care to identify four main patient cohorts who might require action:
- Patients with test results that indicate CKD who might need re-testing and coding
- Patients at high risk of developing CKD who fall within NICE recommendations for screening via eGFR and uACR, but have not been screened in the last 12 months
- Patients on the CKD register who have not been reviewed in the previous 12 months and might need to come to the surgery for an eGFR, uACR and/or blood pressure test
- Patients on the CKD register who might need treatment. Most CKD patients would benefit from optimisiation via ACEi/ARB and Sodium-glucose co-transporter-2 (SGLT-2) inhibitors
Practices can select searches within a cohort to review and decide if patients require further action
